[SFS] free-form, accumulative survey

As pointed out elsewhere this seems to be a text analysis problem coupled
with survey fatigue.
I think there are enough existing SFS courses to start collecting data on
the variables:
* Topic
* Materials
* Location
* Teacher
* Attendees
* Donations

Just my $0.02
